I think I would need to start with an easier version and then work up to the heavy metal.

In high school, I would often spend almost entire weekends doing nothing but gaming, which would result in everyone feeling like a fat slob with a mind of mush by the end. To combat this, my gaming buddies and I began a gamers' training regimen of sorts. After every loss (or death, if we were feeling really crazy) we would draw a card from a standard poker deck. The number value on the card (2 thru 10 , 11 for jack thru 14 for Ace) was the number of push-ups owed and the suit determined the type, i.e. Clubs for fist push-ups, Spades for triceps push-ups, Diamonds for diamond push-ups, Heart for standard or whatever you want. If we were too tired/weakened from too many push-ups we could do sit-ups instead at some agreed-upon exchange rate.

How about instead of doing push-ups for all 4 suits make 2 suits for push-ups and 2 for sit-ups?

No, I just thought it would be fun to have the losing team pull a card and do push-ups as punishment for sucking. It keeps the blood flowing and encourages higher team performance.
Careful there. A lot of people end up liking exercise once they get in shape, so that might not be a great motivation for in-game performance. :P
